Absence Request
As you are aware we are no longer able to authorise any holiday/absence during term times unless there are special circumstances. To see a list of special circumstances please see the attachments below.
Early Leave from school request
If for any reason, apart from a medical appointment, you would like to request leave for your child, this form should be completed and returned to the school office.
Leave of Absence for Medical Appointment
Wherever possible, please try to make medical appointments for your child outside of school hours. Please provide evidence for all appointments which can be letter, medical note, appointment card or screen shot of text. Please also complete and return the form below, to the school office.
Medicine Form
Please be aware that we can only administer medicine that has been prescribed by a doctor. Please bring the medicine in the box with the label with your child’s name on and complete the form below.
Free School Meals
Children in Foundation, Year 1 and Year 2 (Fnd and KS1) all automatically receive the Government Funded Universal Infant Free School Meals (UIFSM). However, to be eligible for Free School Meals (FSMs) and therefore to receive the vouchers, you need to be in receipt of certain benefits (please see the list below).

Universal Credit – if you apply on or after 1 April 2018 your household income must be less than £7,400 a year (after tax and not including any benefits you get)
We encourage anyone, who believes they are eligible, to register for FSMs, even if your child would normally bring a packed lunch or qualifies for UIFSM, as this allows the school to claim additional much needed and valuable funding.
